I had an injured player that I replaced "normally". I made other changes and when I wanted to make a 5th (being convinced that I had made 4) but the game tells me that I have made them all.

Could someone clarify this for me?

You made 5 changes to the injured man plus 4 others

I never realised then, because this screen was at least 5 minutes after the injured player was taken off: why does he always appear as a player on the pitch in the notes?
